Guidelines to register birth of child and death of any person in the concerned Union Council or in Municipal Committee.
1. Duties of Union Council / Municipal Committee
A) Union Council / Municipal Committee shall register births & deaths of people within their jurisdictions.
B) Secretary Union Council / Official of Municipal Committee shall register births & deaths as per prescribed guidelines and they will be responsible for safety of this record. Moreover, they will issue copy on the prescribed proforma.
2. Responsibility to Report Birth and Its Method
A) Family Member or Close Relative will be bound to register the birth of child within 60 days in the concerned Union Council / Municipal Committee
B) To register birth, prescribed "Form A" will be used. This form shall be provided free of cost to the general public.
C) Family Member or Close Relative will submit the filled Form-A in Union Council / Municipal Committee. Secretary of Union Council / Official of Municipal Committee will provide the receipt to the applicant and enter it in the concerned register.
D) If the birth of child occurred out of jurisdiction of Union Council / Municipal Committee then an affidavit will be provided in which proof of child birth should be written along with Form A.
E) Registration of birth will be free of cost. It will also be applied on late register as well.
F) To get computerized certificate, fee of Rs.100-200 (approximately in these days) have to pay.
3. In Case of Late Registration of Birth from 61 Days to 7 Years
A) Applicant would provide Form-A along with affidavit or birth certificate of hospital or school certificate or protective injections card and verified certificate of Councilor or Numberdar.
B) After receiving of application, Chairman / Vice Chairman / Administrator / Secretary Union Council / concerned Official of Municipal Committee would will verify the record.
C) After verification, registration of birth will be done
4. In Case of Late Registration of Birth after 7 Years
A) Secretary Union Council / Official of Municipal Committee will forward the case to Assistant Commissioner (AC) then AC will get the age determined from Medical Superintendent of District Headquarter Hospital. After determination of age, AC will send recommendation to Local Government.
B) After receiving of report, Chairman / Vice Chairman / Administrator will issue order for registration of birth.
C) Secretary Union Council / Official of Municipal Committee will be bound to register it on the same day.
Schedule of Registration of Birth
- Normal Registration in within 60-Days, Responsibility of Secretary Union Council / Official of Municipal Committee
 
- Late Registration from 61-Days to 07-Years, Responsibility of Chairman / Vice Chairman / Administrator Union Council / Municipal Committee
 - Late Registration after 07-Years, Responsibility of Assistant Commissioner for verification of residency, Medical Superintendent of District Headquarter Hospital for determination of age
